Our Policies

Appointment Policy:

We strive to remain on schedule but please understand that emergencies do arise from time to time. Out of consideration to our patients who have scheduled appointments, we do not see "walk-ins" but will accommodate and schedule same day appointments for urgent care, as needed. Please call ahead to schedule an appointment so that we can give you the proper attention that you deserve. If you are late for your scheduled appointment, your appointment may have to be rescheduled. This is in consideration for our other patients who have appointments.

We request that you notify us 24 hours in advance if you cannot keep your scheduled appointment. If you fail to comply with this policy twice, you will only be allowed to book "same day" appointments. This means that you can call in the morning to make an appointment to be seen by a physician later that day, as available.

List of items to bring to your appointment:

  • Insurance cards & driver's license
  • Co pay
  • Current medications
  • List of questions you have for the physician to help maximize your time
  • Names, addresses, fax numbers of doctors you want records sent to
  • New patient paperwork, if not returned prior to your appointment

Financial Policy:

Payment is due at the time of service, including your co-pay or deductible. If you do not have insurance, please call us before your appointment to discuss payment arrangements. We accept cash, debit cards, Visa, or Mastercard.

Our practice participates with many insurance companies/networks. Please refer to the Hospital/Insurance page for a complete listing.

Past due balances are initially handled in our office. Accounts with balances past 60 days are subject to an APR of 12%. Accounts that are unpaid after 90 days will be subject to credit agency reporting. This will affect your credit rating and your medical care with this practice.

Medical Records Policy:

Our practice cannot send or receive medical records to or from another medical office without the patient's written authorization.

To obtain a copy of your medical records from our office, there will be a $10.00 handling fee, which also includes shipping costs, if necessary. In addition, there is a 50 cent charge for each of the first 50 pages printed. Each page thereafter costs 25 cents.
